What is the cost of testing?
Our testing and evaluation services are self pay only. This approach allows us to provide more personalized and comprehensive assessments without the limitations often imposed by insurance companies. It ensures that we can focus on delivering the highest quality of care tailored to each child’s unique needs, leading to more accurate diagnoses and effective treatment plans. Do I need a referral?
CCCofH accepts self or caregiver referral for testing; however, your primary care doctor or school may make a referral. It is likely that the evaluator will seek input from such outside resources.
Release of Reports in Cases of Divorced or Separated Parents:
If parents are divorced or separated and only one parent is assuming the financial responsibility for an assessment, the following policy applies:
- The parent who has paid for the assessment will receive a copy of the report as part of the service.
- If the non-paying parent requests a copy of the report, they will be required to:
- Schedule and attend a separate feedback session, the cost of which will be their responsibility.
- Pay a fee for the copy of the report, which will be based on Pennsylvania’s guidelines for copying medical records and may include charges for labor, materials, and postage.
- Both parents retain the right to access the child’s medical records unless restricted by a court order or custody agreement. A copy of any such order or agreement must be provided to the practice.
